To make an appointment
with one of our doctors please call our office at
(402) 721-0090 between 8:30 a.m. - 5:00 p.m., Monday - Friday.
To make an appointment with Dr Ihle at Schuyler location call (402) 352-2441.
Before you are seen by the doctor:
You will be asked to fill out 2
forms which
ask for your personal information, your insurance policy
number and your signature on an assignment of benefit. The
office charge for your first visit will probably be somewhat
higher than for follow-up visits. Your visit may include
additional charges for services such as bracing, x-rays,
and supplies.
Here is a convent check list of what
will help your doctor help you:

Please
complete Insurance and Medical forms below and present
them to our receptionist at the time of your appointment |

Please
bring any medical records to your appointment as
well as the actual x-ray films, MRI films or CT scans
that you have had recently. |

Your
current insurance card is required and will be copied
at the time of your first visit. |
|
If
a referral authorization is needed for your appointment,
it is your responsibility to bring a written authorization
with you. |
|
If
your insurance requires a co-pay this is expected
at the time of checking in. |
Patients are seen by appointment only:
You will be given the first available appointment. We will
make every effort to see you as soon as possible. If you
are scheduling a post-operative evaluation please contact
the office soon after your discharge from the hospital and
notify our receptionist that this will be a post-operative
visit.
Please schedule your appointment with the doctor of
your choice and we will try to schedule all subsequent appointments
with the same physician. If your physician is not available
at the time of your visit, or if you wish to see a different
physician, you are welcome to schedule an appointment with
another doctor in the office.
